For the Vitality Experience you need to schedule your program consult. If you are not sure, just send us a request for a proposal!
Appointment Details
All consults are done on Telehealth by video or phone. Infusions and Injections will be scheduled in-person at the Vitality Lounge after the initial consult. Once you complete your initial intake forms and request you will be sent further information on onboarding and enrollment.
- You must be 18 and older and be able to sign your own consents for regular consults
- Sports Physicals are $15 (ages 12 and older)
- We do not accept any Insurance plans
- You can submit your visit invoice to your insurance or HSA account
- Your orders, labs, or local pharmacy prescriptions might be covered by your insurance if accepted
- You must be located in a state we service
- IV infusions are staffed based on your payment. You will be sent a separate scheduling link with available dates and times. If one is not available, you can send a request to your patient portal
Getting On Your Charm Portal
Prior to your visit:
- Complete your health forms that will be sent to your email. Check your spam
- We use Charm health portal to provide a secure connection with your personal information
- You will need to join the patient portal to get the best communication with your Vitality Team
