Yes. If you provide medical history or PHI through our intake, we use HIPAA‑compliant processes and vendors for storage and transmission. For non‑PHI lead capture (name, phone, email, BMI range), a standard contact form may be used; for clinical intake that includes PHI we use a HIPAA‑capable form provider and a Business Associate Agreement when required.

Our providers DO NOT fill out forms or do prior authorizations.

We take your privacy seriously. That is why we send you directly to our HIPPA compliant secure scheduling and Telehealth Platforms.

A nurse practitioner (NP) is a registered nurse (RN) who has completed advanced education (a minimum of a master’s degree) and training in the diagnosis and management of common medical conditions, including chronic disease, mental health, and acute illness. A nurse practitioner can serve as a patient’s regular primary care provider (PCP). At Vitality Wellness our nurse practitioners are trained to take care of your complex individual health care needs. Our nurse practitioners can prescribe medications, order testing and labs, and make referrals if needed.

The program includes a medical evaluation, baseline labs, clinician‑supervised medication management (when appropriate), individualized nutrition counseling, behavioral coaching, and scheduled follow‑ups for safety and progress monitoring.

Usually no referral is required for an initial evaluation, but we encourage coordination with your primary care provider. If your insurance requires a referral for coverage, we will guide you through that process.

Bring a list of current medications, recent lab results if available, insurance card, and a photo ID. If you have prior authorization paperwork or prior records from another clinic, bring those or upload them securely before the visit.

Cancellation and refund policies are provided at booking and in the patient intake materials. Medication orders may be subject to manufacturer return policies and pharmacy rules; we explain these details during benefits checks and prior‑authorization steps.

Medication and Clinical Questions

Contact support for additional questions or concerns. support@ivitalitycenter.com

Wegovy (semaglutide), Zepbound (tirzepatide), and Saxenda (liraglutide) are FDA‑approved for chronic weight management.

Ozempic (semaglutide) and Mounjaro (tirzepatide) are FDA‑approved for type 2 diabetes; both have been associated with weight loss in clinical practice and trials, but their labeled indications differ from the weight‑management formulations. Clinicians will explain which product and dose are appropriate for each patient.

Duration is individualized. Some patients use medication long term to maintain weight loss; others reassess goals and may taper under clinician supervision. Stopping medication can lead to weight regain for some people, so long‑term planning and lifestyle support are important.

Common early side effects include nausea, vomiting, constipation, and transient GI upset. Most side effects improve with gradual dose titration and clinician support. Serious but rare risks (e.g., pancreatitis, gallbladder issues) are monitored with history, exam, and labs; report severe symptoms immediately.

Monitoring includes scheduled follow‑ups, symptom review, periodic labs as clinically indicated, and dose adjustments. We document baseline labs before initiation when appropriate and repeat testing based on medication and medical history.

Vitality Wellness Center uses FDA‑approved formulations and follows current safety guidance. We do not recommend or dispense unapproved compounded copycat products.

Individual responses vary. Some patients notice appetite reduction and early weight changes within weeks; clinically meaningful weight loss typically accumulates over months. Your clinician will set realistic milestones and monitor progress.

Many patients with type 2 diabetes are treated with GLP‑1 or GIP/GLP‑1 agents, but suitability depends on your full medical history, current medications, and lab results. A clinician will evaluate risks and benefits during the medical assessment.

Understanding Your Blood Tests

Includes:

  • CBC: Is a group of blood tests that measure the number and size of the different cells in your blood. A CBC measures: Red blood cells, which carry oxygen from your lungs to the rest of your body. White blood cells, which fight infections and other diseases.

Includes:

  • Kidney Function: This is an evaluation of how your kidneys are functioning. The kidneys’ job is to filter the blood. Medications that are filtered by the kidney can accumulate in the blood if kidney function is not normal. A blood urea nitrogen (BUN) test measures the amount of urea nitrogen in your blood and is an indicator of the kidney’s ability to function as a filter. Creatinine is a compound produced by the muscles in your body and, like urea nitrogen, is filtered out of the blood by the kidneys. Measuring both of these levels can give your provider an idea of how your kidneys are functioning.
  • Liver tests: Show substances made by the liver. Abnormal results can indicate liver inflammation, fatty liver, or damage to the heart, bones, or muscles. It can also be from medications, liver injury due to substance like alcohol consumption or hepatitis.

Includes:

  • Lipid Panel: Helps to determine risk factors for cardiovascular disease and stroke. High LDL cholesterol or “low density lipoproteins” sometimes called “bad cholesterol” can indicate greater cardiovascular risk. Low HDL cholesterol or “high density lipoproteins” sometimes called “good cholesterol” can be similarly concerning for higher cardiovascular risk. High values of cholesterol and triglycerides, similarly, have been shown to be associated with increased risk of cardiovascular disease.

Includes:

  • Thyroid stimulating hormone (TSH): Your thyroid is an important gland that helps regulate metabolism. A high or low level could indicate metabolic dysfunction.

Includes:

  • Hemoglobin A1c: A1c is a measure of the long-term (3 month) history of your body’s ability to manage blood glucose. Elevated A1c can indicate problems regulating blood sugar levels, which is important for good health. An A1c 5.7-6.4 is an indication of Pre-diabetes which can lead to type 2 diabetes if left untreated. A1c higher than 6.5 is considered Type-2 diabetes.

Includes:

  • PSA: Protein produced by the prostate gland that can be measured in a blood test to screen for prostate cancer.  An elevated PSA level may indicate the presence of enlarged prostate which is normal with age or prostate cancer. Though other factors can also cause elevated levels including prostate infections.
